I can't quite make out the minefield pictures but the ship pics show you have correct friendly code and suitable HYP distance waypoint set. You then moved 15 LY before striking a web. Odds of moving at least 15 LY before web strike were 46% (0.95^15 = 46%).
As to why you didn't hyp, you show NO decay for the web in the small second picture where you did show decay in the small first picture. This implies to me that you did NOT have a nearby ship minesweeping THIS turn so there could be a second web and you wouldn't see it. Are you sure that you have current turn minesweep mssgs? Also, we don't see the center of the web, is it covered in an ion storm so you can't actually sweep it or scan it?
